SMM: This Summary of Material Modification (SMM) is issued to amend the current Summary Plan Description (SPD). The following text has been added to the Glossary section of the SPD. Subrogation was included in prior versions of the SPD but was inadvertently omitted from this version until this admendment.
Subrogation is the process of collectiong payment for medical and pharmacy claims from a member or an outside third-party payer in situations, including but not limited to, auto accidents, workers compensation, third-party liability or personal injury claims resulting in legal judgements. Members are required to cooperate with the Plan's subrogation process to collect for services that are the legal responsibility of a third party or to personally reimburse the plan for any paid claim that is later paid for or settled by a third party.

SMM: This Summary of Material Modification (SMM) is issued to amend the current Summary Plan Description (SPD) and is intended to change the existing enrollment deadlines for members with family coverage who wish to add a dependent after a birth or adoption, described on page 9. This clarification was adopted by the State & Public School Life & Health Insurance Board on November 21, 2007:
Birth. Provide change form to insurance representative to add child within 30 days of date of birth.
Adoption. Provide change form to add dependent to insurance representative within 30 days of placement or filing a petition for adoption.

SMM: This Summary of Material Modification (SMM) is issued to amend the current Summary Plan Description (SPD) and is intended to clarify the existing Exclusion and Limitation for Obesity Therapy or Treatment described on page 40. This clarification was adopted by the State & Public School Life & Health Insurance Board on January 16, 2007:
Obesity therapy or treatment. All surgical interventions for the treatment of obesity, such as gastric bypass or lap band procedures are not covered services under this plan. Certain non-surgical obesity treatments are available through the wellness program, ARWellness.

CORRECTION! Error on Page 7 of EBD Buzz
On page 7 of the Vol. 3, Issue 1 edition of the EBD Buzz, a statement reads "All health plans include one vision screening every 24 months and dental screenings (including cleanings, x-rays, and oral exams: flouride treatments are covered for dependents under age 19) twice a year." This statement is incorrect and this applies to HMO & POS plans only. This statement has already been corrected on the web version of the EBD Buzz. We apologize for any confusion this may have caused.

Important Update! Forteo Error on PDL
The current Preferred Drug List (PDL), dated 10-01-2006, shows the drug Forteo listed as a second tier drug with no indication of it requiring a prior authorization. This is a 2nd tier drug; however, it DOES require a prior authorization. It has since before our switch to NMHC on October 1, 2004. The PDL will be corrected with an indicator of * (PA) the next time we do an update. We hope to have the next update around April 1, 2007.

The following SMM is issued as a modification of the second instance as to when a Special Enrollment Period would occur (the bolded section has been added for clarification) as read from page 54 of the Summary Plan Description:
SMM: Special Enrollment Period:
• After the addition of a dependent – A special enrollment period occurs for an employee, employee’s spouse, employee’s current dependent child(ren), or employee’s new dependent child:
(a) after the employee marries,
(b) after an employee’s child is born, or
(c) after an employee adopts a child or has a child placed with the employee for adoption.
